Restaurant Food Safety Incident Log Bangladesh: Contain, Trace and Close
A practical incident form that records the first facts, protects affected food and people, preserves traceability and proves corrective action closure.

Last verified: 2026-08-31
A restaurant food safety incident log records what was known, what was contained and who made each decision. Open one for suspected contamination, allergen error, temperature failure, foreign object, chemical exposure, pest contact, unsafe water, employee illness concern or a guest complaint that may indicate foodborne illness.
The first priority is safety, not completing a perfect form. Stop unsafe service, isolate affected items, protect people, preserve evidence and escalate. Add verified facts as they emerge, keeping initial reports separate from later conclusions.
Immediate action card
Give one person incident command and another person responsibility for contemporaneous notes. Time-stamp every action.
If there is immediate danger or medical need, follow emergency and authority direction before routine documentation.
| Action | Record | Do not |
|---|---|---|
| Stop | Product/process/equipment paused and time | Continue service to avoid waste |
| Isolate | Item, batch, quantity and location | Mix it with usable stock |
| Protect | Guest/staff assistance and emergency contact | Diagnose illness yourself |
| Notify | Duty manager and required specialists/authority | Wait for the next shift |
| Preserve | Labels, receipts, temperature, photos and sample as advised | Discard evidence without direction |
| Trace | Supplier, preparation, KOT/order and recipients | Rely on memory |
Capture the first report without leading the reporter
Record reporter name and contact, date/time, exact words, food or event described, consumption time, symptoms or observation, other affected people and how the restaurant was contacted. Do not suggest a cause or promise a conclusion.
For a guest report, be respectful and collect only necessary information. Restrict health and contact data to authorised staff and follow professional guidance on retention and disclosure.
Define the potentially affected scope
Start broad enough to prevent further exposure, then narrow only with evidence and competent advice. Record why the scope changed.
Keep quarantined stock visibly identified with incident ID, quantity, location, date and release/disposal authority.
| Trace point | Questions |
|---|---|
| Ingredient | Supplier, lot, delivery and remaining stock? |
| Preparation | Batch, recipe/process, time and staff? |
| Equipment | Unit, setting, calibration and fault? |
| Service | Orders, tables, delivery IDs and time window? |
| People | Handlers, guests and other reports? |
| Environment | Water, pest, chemical, power or sanitation event? |
Preserve evidence and chain of custody
List every photo, label, invoice, temperature record, cleaning log, staff roster, POS order, marketplace order and sample. Record who collected it, when, where it is stored and each transfer.
Do not create missing historical readings after the event. Mark estimates as estimates and source them to the person who provided the recollection.
Notification decision log
Record who assessed whether to contact BFSA or another competent authority, supplier, platform, insurer, landlord, laboratory, affected customers or legal adviser. Capture the decision, time, basis and any reference number.
The template cannot decide mandatory reporting. Use the current Bangladesh requirement and direct authority or professional guidance for the actual incident.

Investigate cause without confusing it with fact
Separate confirmed facts, working hypotheses, tests and conclusions. Ask what control should have prevented the event, whether it existed, whether it was followed and whether the record is reliable.
Use categories such as supplier, storage, preparation, cross-contact, temperature, sanitation, equipment, water, pest, packaging, transport and human factors. A person may be involved without being the system root cause.
Corrective and preventive action table
Training alone is not a sufficient answer when layout, equipment, supplier or workload caused the failure. Choose an action that changes the control.
| Field | Example content |
|---|---|
| Immediate correction | Affected stock quarantined |
| Root cause | Verified cause, not assumption |
| Preventive action | Process, equipment or training change |
| Owner/date | Named accountable person and deadline |
| Verification | Observation, test or record proving action works |
| Release | Who authorised reopening/product release |
| Follow-up | Trend review date and result |

How we calculated and verified this
The BFSA source anchors the Bangladesh restaurant context. The FDA Food Code is cited only as a clearly labelled international model for retail-food controls, not as Bangladesh law.
This incident log is an operator template. Suspected illness, contamination, injury, chemical exposure or authority-reporting questions require immediate competent and local-authority guidance.
A food-safety incident record should make the response faster and more reliable, not delay it. Preserve facts, control the affected scope and close only after preventive action is verified.
